Each year we strive to keep you updated on the latest advancements in our field and this year is no exception. With MDS 3.0 and RUGs IV on the horizon, a continued increase in QIS surveys, new guidelines for unintended weight loss and the need to implement the nutrition care process in our documentation, we face many challenges as we embrace the ever changing regulations and recommendations. In addition, we must continue to move forward on our culture change journey by offering liberalized diets and dining choices to those we serve.

10:15 - 12:15 p.m. General Session - MDS 3.0 - It's a hard act to 'Swallow' - Robin Hillier

The MDS 3.0 is scheduled for implementation by CMS for skilled nursing facilities on October 1, 2010. Because this version is drastically different than the MDS 3.0 facilities need to take every opportunity to educate their staff on the changes. This session will identify the changes in Section K of the MDS 3.0 from the MDS 2.0. Section K requires input from several team members to assure accuracy in coding and care planning. The intent of the Swallowing / Nutritional Status section is stated in the RAI manual as such: The items in this section are intended to assess the many conditions that could affect the resident's ability to maintain adequate nutrition and hydration. This section covers swallowing disorders, height and weight, weight loss, and nutritional approaches. The manual recommends collaboration in this section from the dietitian and dietary staff to ensure that items have been assessed and calculated accurately. Section K has a 7 day look-back for identification of among other things swallowing problems. Information is gathered through Assessment, Asking the Resident, Observation and Consultation with interdisciplinary team members and family. Attendees will learn how to define the changes in coding on the MDS 3.0, and describe how to incorporate standards of practice within your facility. A must attend for the entire dietary department.

1. The Obesity Challenge: Aging, Obesity and Long Term Care - Becky Dorner, RD, LD, President, Becky Dorner & Associates, Inc.

An astonishing 66% of Americans are overweight or obese, and most states boast an obesity rate of a whopping 20% or more. The growth rate of our waistlines is staggering, and along with it comes increased rates of diabetes and other chronic illnesses. Panel presentation

Innovations in food, nutrition and dining including liberalized diets, choice menus and “refrigerator rights” have been welcomed by providers but present challenges when integrating new approaches with the regulatory guidelines. This session will explore the key highlights from the Pioneer Networks 2010 National Symposium on Culture Change and the Food and Dining Requirements. A panel of Ohio food and nutrition professionals will come together and discuss the following topics and outline potential barriers & solutions:

- Expanded Roles of the Medical Director and Pharmacist in Nutrition Care

8:30 a.m. - 9:30 a.m. RUGs IV or Not?

Medicare uses a case-mix classification system to assign a nursing home resident to a RUG category based on his or her medical conditions and the resources needed to provide care. Each RUG category is tied to a Medicare payment rate. All of the changes between RUG III and IV will be reviewed in detail. Understanding RUG IV is essential to facility success, in this session the impact of the Dietary sections to RUG IV will be a particular focus.

As dietary professionals we are faced with numerous surveys of our kitchens from local county inspectors to the state surveyors. The thought of the survey is enough to insight panic among some, so it is best to be prepared. F – Tag 371 remains in the top 2 citations in the state of Ohio as well as the nation overall. What are the other top citations? This informative session will highlight the issues facing our department today. This presentation will also focus on the changes in the survey process with the Quality Indicator Survey (QIS) regarding the foodservice department, as well as dietary assessment and care plans.

3. Unintended Weight Loss in Older Adults: ADA Evidence Based Practice Guidelines

This session provides an overview of the Evidence-Based Nutrition Practice Guideline on Unintended Weight Loss (UWL) developed to make defendable decisions about nutrition care choices in health care settings. Practitioners will be provided with evidence to screen and assess adults over 65 with UWL; diagnose UWL; plan appropriate Medical Nutrition Therapy (MNT); and evaluate and monitor MNT intervention. The guideline debuts the ADA Evidence Analysis Library's first nutrition diagnosis recommendation.


4. Sustainability Fundamentals: How To Be Green While Saving “Green$”

Hydration and Dysphagia. The challenge lies in the resident with impaired swallowing function. We strive to provide optimal hydration and the least restrictive diet modifications possible, yet are often faced with the dilemma of the noncompliant resident refusing mechanically altered diets and thickened liquids. What tests should be performed and what are the latest treatments to work with the dysphagia resident? This panel of experts will address these issues and concerns from all angles including recommended studies and treatments, the latest products available to address diet modifications and the Frasier protocol.

Jolene Barrett is the Corporte Dietitian for HCF Management, Inc., located in Lima, Ohio. She has a Bachelor of Science from Kansas State University in Dietetics, Restaurant, and Institutional Management. She completed her post - graduate work at The Ohio State University to become a Licensed Nursing Home Administrator. She has over twenty-five (25) years iexperience in long Term Care. She has served on the American Dietetic Association Commision of Dietetic Registration. She authored the Deaconess Hospital Surgical Weight Loss Manual, and the Geriatric Section of the Cincinnati Diet Manual. She is currently a National Restaurant Association Serv-Safe Instructor and is a member of the OHCA Dietary Council.

Phil A Bushey comes to our panel from Hennis Care Centre of Bolivar. Phil has over 36 years experience as a Speech Pathologist in geriatric care for nursing home residents. Phil received his Bachelor’s degree in Speech from Malone University in 1970 and his Masters degree in Speech Pathology from Kent State University in 1974.
Phil was in private practice for over 20 years, and has developed and coordinated speech/language/swallow programs in Community Clinics, Skilled Nursing Facilities, Home Health Agencies and Hospital Dietary Programs. Phil has worked closely with Dietitians in many settings helping with hydration/nutrition efforts and Dysphagia Rehab programs as well.
At Hennis the Frazier Water Protocol has been utilized in the over all hydration/nutrition programming for their residents.

Barbara Christensen, DTR has over 20 years experience as a restaurant manager, Certified Dietary Manager and Dietetic Technician, Registered. She’s been a clinical consultant for a contract management company as well as a Food Service Director; primarily in long term care settings. Currently she’s the Central Region’s Operator Sales Manager for Hormel Health Labs, a division of Diamond Crystal Sales. In 2000 Barbara was awarded the Recognized Dietetic Technician of the Year for Ohio. In 2005 she received the American Dietetic Association Award of Excellence in Dietetic Technology. She has been the President of the Columbus Dietetic Association and a Delegate-at-Large for the ADA House of Delegates.

Suzanne Cryst RD,CSG,LD is currently Director of Nutrition Services at the Maria Joseph Center, a 305 bed facility in Dayton, Ohio, and has worked as a full time licensed registered dietitian in long term care for over 30 years. She has served in many volunteer postions with the American Dietetic Association (ADA) and is currently president of the Ohio Dietetic Association, Network Representative from Dietetics in Health Care Communities ( a practicve groups of ADA) to the American Medical Directors Association and is a member of the Ohio Medical Directors Board. She has received the Excellence in Management Award from ADA Foundation and the 2008 AMDA/Evercare Award in Quality Improvement.

Becky Dorner, RD, LD, President, Becky Dorner & Associates, Inc. Widely-known as one of the nation's leading experts on nutrition and long-term care issues, Becky Dorner has almost 30 years experience as a consultant, author and speaker. She is President of Becky Dorner & Associates, Inc., which publishes and presents CEU programs and information on nutrition care for older adults; and Nutrition Consulting Services, Inc. which employs RDs and DTRs to provide services to health care facilities in Ohio (since 1983). Becky's mission to improve care for older adults has inspired her to present more than 500 programs for national, international and state professional meetings in 5 countries and 48 states; and to publish more than 225 manuals, CEU programs and practical articles for professional journals and newsletters. Her free email magazine keeps more than 25,000 health care professionals up to date on the latest news in the field. A 20+ year volunteer to ADA and other professional organizations, Becky has held positions as CD-HCF chair, ADA Delegate and NPUAP Officer. She has served on the ADA Research Committee, the AODA Leadership Team and is currently on the board of the National Pressure Ulcer Advisory Panel. Honors include: ADA Award of Excellence in Business and Consultation, NE Outstanding Nutrition Entrepreneur, ADA Recognized Young Dietitian of the Year and CD-HCF Distinguished Member Award.

Charlette Gallagher-Allred, Retired, Former Director Professional Relations & Education, Abbott Nutrition, is an international nutrition and healthcare consultant. She has been a developer of nutrition care alerts for nursing home residents and home care patients, developer of best practice guidelines for pressure ulcer prevention and treatment, nutritional status screening and assessment, and tube feeding. She has also directed a collaborative effort of 25 national organizations to set national standards for a streamlined, cost-effective accreditation process for sub-acute, long-term care, assisted living, home care, adult day services, homemaker services, and hospice care. A member of the ADA Evidence-Based Nutrition Practice Committee Guidelines, UWL Workgroup and author of 2 books and over 50 professional publications. She lectures frequently on nutritional care of the terminally ill.

Robin L. Hillier, CPA, STNA, LNHA RAC-MT, is a certified public accountant, state tested nurse aide and licensed nursing home administrator who is also resident assessment coordinator certified and a master trainer through the American Association of Nursing Assessment Coordinators. She is President of RLH Consulting, which provides reimbursement, accounting and operational consulting to long term care providers. She is Immediate Past President of EFOHCA and a Past President of OHCA and is a member of the AHCA Future Leader class of 2007. She currently co-chairs the OHCA MDS Task Force, chairs the Convention Task Force and serves on many other committees. RLH Consulting is an OHCA Platinum Champion Partner. Robin was invited by CMS to attend the National CMS Train-the-Trainer training on the MDS 3.0 process. Robin served as one of two faculty members of the OHCA Comprehensive MDS 3.0 Training conducted in 10 locations throughout the state.

Kim Hofmann, RD., LD, is the owner of Get to the Point-Presentations With Purpose. She draws from her extensive real-world experience to develop and deliver programs that give practical knowledge, tips, tools, and skills needed to succeed in today's fast-paced business environment. Kim has over 24 years experience in management, training, sales, and food service operations. Cathy Johnston MBA, RD, LD, Director, Nutrition Services, The MacIntosh Company - Cathy has been the Director of Nutrition Services for the MacIntosh Company in Columbus, Ohio for the past 11 years. She provides regulatory and operations oversight for all aspects of food and nutrition services to 5 nursing, skilled and assisted living communities in central Ohio. Prior to that she operated her own dietary consulting company and has worked with over 50 long-term care and retirement facilities. She received her B.S. degree in Medical Dietetics from the Ohio State University and recently completed her Masters in Business Administration from The Capital University.

Jeff Joiner has a Bachelor's Degree in Communication Arts from Cedarville University and a Master's Degree in Speech Communication from Miami University. Jeff taught on the faculty of Miami University for 5 years where he coached their championship speech team to 3 State and 2 International Championships. He has been recognized by both the Ohio State Senate and Ohio House of Representatives for excellence in teaching, coaching, and team-building. Jeff worked for the Campbell Soup Company for 10 years, where he was awarded Campbell’s prestigious Merit Award for Excellence three times. He has worked with hundreds of foodservice organizations to improve their profit, employee satisfaction, and workplaces. Some of the organizations for whom Jeff has provided training include: Amerinet, The Health Alliance, Advance Food Company, The School Nutrition Association (SNA), the Dietary Managers Association (DMA), the National Association of College & University Food Service (NACUFS), the Ohio Association of School Business Administrators, The Kentucky Association of School Foodservice Administrators, the United States Air Force, SYSCO, Gordon Food Service, US Foodservice, as well as hundreds of school districts, universities, and commercial operators. Jeff now works for Midwest Regional Sales, the Midwest region’s premier food brokerage organization. Amy Kotterman RD,LD graduated from The Ohio State University in 1990 with a Bachelor of Science degree in Medical Dietetics. She is the Corporate Dietitian for United Church Homes where she has been for 19 years. She is a member of The American Dietetic Association, Dietetics in Health Care Communities, the current chairman of the Ohio Health Care Association Dietary Council and a former Examiner for the Ohio Award for Excellence. Amy has spoken on Person Centered Care and Enhanced Dining programs at the Person Centered Care Coalition conference, as well as at previous OHCA conferences.

Maureen Leugers, MBA, RD, CD is the Director of Healthcare Marketing for Gordon Food Service. Offering strong operational knowledge as well as nutrition expertise, Maureen’s background includes the role of the Division Director of both Environmental Services and Nutritional Services at a large community hospital. She is a former instructor in the Business Department at the University of Saint Francis. Maureen received her MBA from Saint Francis College and has a BS in Dietetics from the College of Mount Saint Joseph. She is Past President of the Indiana Dietetic Association. On the National level, Maureen is currently a member of the Industry Advisory Board for AHF, the Industry Advisory Council for DMA, and serves on the Board Of Directors for Elder Law of Michigan and Providence Healthcare. Her dedication to the healthcare foodservice industry and the profession of dietetics earned her the 2003 Honored Dietitian of the Year Award from Indiana Dietetic Association, and the 2008 Allied Member of the Year Award from ASHFSA.


Susan Marsico, LNHA, Director of Training, HCF Management, Inc. has over twenty five years of experience in long term healthcare. She has been a licensed nursing home Administrator for 22 years and has extensive experience in the areas of Person Centered Care, leadership, employee development, customer service, public relations, and quality of life issues. She has had consulting relationships, and provided training programs for colleges, industry and healthcare organizations. Susan is the Director of Training for HCF Management, Inc. She is also an Eden Alternative Associate, and holds a B.S. in Gerontology from Bowling Green State University. Susan is the co-author of LTC: Leadership Training and Conditioning which develops Person Centered Leaders, and is co-owner of the consulting group, Leeson Brown, Ltd.

Gretchen Robinson, Corporate Consultant, HCF Management Inc. Gretchen Robinson’s dedication to quality of life for the elderly is paramount to her professional career as Corporate Dietitian for Health Care Facilities, Inc (1978-2004).Currently she works as a consultant to 3 LTC facilities.A lecturer, writer and consultant Gretchen utilizes interdisciplinary associations with health practitioners to enhance the role of the dietitian.As a HOD delegate she facilitated 6 workshops on the Nutrition Care Process training over 300 RDs and DTRs throughout Ohio.Gretchen served on a task force that recommended changes to the Ohio Board of Dietetics and aligned licensure rules with the ADA SOP/SOPP. She is co-chair of DHCC committee developing practice guidelines.A member of ADAs evidence based guideline “Unintended Weight Loss in Older Adults” brings full circle her leadership abilities to think strategically, flexibility to adjust strategies and creativity to seek out new opportunities for the RD and DTR.

Dr. Mike Thomson has extensive professional experience, including Supervision in the Department of Psychology and Psychiatry at The Mayo Clinic, Director of healthcare centers in Minnesota, Ohio and Sweden, an Adjunct Associate Professor with The Ohio State University and Ashland University, Certified Reality Therapist and President of two successful companies, Thomson ETC., Inc. and It’s All About Character, Inc. Dr. Mike, as he is known around the country, is the host of the award winning television series “In Search of Character” as well as an author of “Strategies for Saving Your Sanity In The Classroom,” “Strategies for Saving Your Sanity In Parenthood,” “Getting Your Parents Off Your Back…And On Your

Maureen Wern has been in the long term Industry for over 30 years. During that time she has served in several capacities including executive director of a 150 bed SNF with an adjacent 75 bed ALF. She is the President of Wern & Associates, a long term care consulting firm and has been a member of OHCA since the early eighties. Currently, Mrs. Wern serves as a Board member of the OHCA. Mrs. Wern is also a member of the Executive Committee of the Board of Directors. She is the Co-chair of the MDS Task Force and the Chair of the Education Committee for OHCA. She has served on both the DAVE task force and the STRIVE committee at CMS. Maureen has been a Master instructor for 7 years with the NASPAC MDS certification program. She also served on the NASPAC Board of Directors for 6 years. Maureen was invited by CMS to attend the National CMS Train-the-Trainer training on the MDS 3.0 process. Maureen served as one of two faculty members of the OHCA Comprehensive MDS 3.0 Training conducted in 10 locations throughout the state.

Ohio Licensed Administrators: The Ohio Health Care Association is an approved provider of continuing education credit by the Ohio Board of Examiners of Nursing Home Administrators. Administrations can earn 13 hours of continuing education credit.

Chefs:
Credit has been approved from the American Culinary Federation.

Dietary Professionals:
Credit has been approved from the Certified Board for Dietary Managers. Credit is pending from the Commission on Dietetic Registration.

Nurses:
*Nurses please note any continuing education that has been approved by BENHA (or any other accredited body) can be accepted by the Ohio Board of Nursing in the State of Ohio. Nurses may use this continuing education to meet their licensure requirements. Please refer to OBN continuing education rules 4723-14-01 thru 4723-14-19.
